Request Counselling

A decision to ask for help through counselling is an important one. It requires a partnership between you and your counsellor. In order for us to best assist you in your journey, please follow the instructions for completing the forms below.

There is an option to complete the forms online, or by downloading a PDF copy which can be printed, and sent as an attachment to [email protected]

Once all required forms are completed and received, we will contact you about an appointment.

For Non-Members of Redemption Church

We are glad to meet with members from other churches, age 18 and older, but we do require the Church Recommendation Form to be completed by your Pastor or Church leader, and that they also recommend a spiritually mature member from your church to attend the counselling sessions as your Advocate. Both of these forms are explained in more detail below and are required before counselling can be scheduled.
